Recognizing the importance of learning collaboratives in supporting continuous quality improvement, the RHP 9 Learning Collaborative Plan reflects the opportunities for shared learning in our region. The overall goals of our learning collaboratives are to:
- Present best practices and project updates
- Capitalize on shared learning and collaborations
- Support continuous quality improvement
- Ongoing communication
- Regional performance monitoring
- Strengthen the DSRIP projects by continuing conversations with Medicaid MCOs for possible collaborations and integration
- Plan for longer term extension through a focus on sustainability, value-based purchasing and alternative payment methods.
- Continue to transform the health care delivery system towards achievement of the Triple Aim.
The RHP 9 Learning Collaboratives provides a forum through which all performing providers can engage in and improve the performance of the region as a whole with respect to the RHP 9 Plan. It provides the forum for sharing successes, working through challenges, entertaining ideas and suggestions and raising concerns.
We continue to encourage the use of PDSA and will also share Rapid Cycle Improvement methodologies during DY 7-8.
We will continue to host cross regional Learning Collaborative events with RHPs 10 and 18. We continually seek out ways to involve other RHPs at our events. With all our events we reach out to providers in the state, MCOs, stakeholders, and community partners.
The takeaways are based on the topics for each event. But overall the events are structured to provide the tools, knowledge, and pathways to sustain and further achieve outcome measures goals and regional transformation.
The Learning Collaborative format and frequency are based on our goals and the needs / requests of our providers.
